1. Download the UPCM scholarship application form. Please click on the link below.
2. Fill-out the application form and submit together with the COMPLETE requirements in 1 pdf file through
email at sspascual1@up.edu.ph
Filename Format: Complete Name (Last Name, First Name, Middle Initial)
Application Period:
- UPCM Scholarship May 9, 2022 until June 30, 2022
- No DOH Scholarship application for AY 2022-2023
