What Information We Collect

The College of Medicine advocates data privacy and ensures that the information required is necessary for the application to perform its functions. No information will be collected from our users until they have agreed to the terms and conditions provided in the privacy policy and have given their consent. To visitors, we collect your email, first name, last name, sex, age which is optional, contact number and city/province. The email is used to deliver the unique QR code generated that is available within that day. The name, sex, age, and location are collected for demographic purposes. The contact number is collected as an alternative way of contacting the person besides the email address. To visitors and UPCM Web Portal users, the temperature, symptoms experienced, if you’re exposed to a person with COVID-19 and purpose are collected for records.

When a visitor or a UPCM Web Portal user visits an office or gate, the guard or office representative scans your QR code to log the location and when you have visited.

Why We Collect Your Information

  • We collect your information to enable you to report your health condition
  • To do secondary screening whenever you are experiencing symptoms
  • To send you updates and alerts via email so that you can be informed and you may view previous information such as the visit date and the QR code
  • Information collected via the electronic logbook QR code function is pivotal for the college to conduct proper and efficient contact tracing.
  • To ensure that the College of Medicine website works as it should and you are able to browse the website safely and securely. We also collect your information to track bugs, errors, and usage statistics. In case of any security incident or data breach, we may also use your information in our investigative reporting to the National Privacy Commission
